This is an exciting opportunity for a 12-month senior leadership secondment developed in partnership with the West Yorkshire Health and Care Partnership Community Collaborative and hosted by Bradford District Care Foundation Trust. We are looking for a dynamic, system focused clinical leader to lead on the strategic planning of a West Yorkshire wide approach to the development of a symptom based, solution focused rehabilitation solution for complex conditions such as Long Covid, ME/CSF and long-term conditions. Using senior leadership experience and advanced clinical knowledge of rehabilitation and personalised care, our aim will be to propose a model(s) of care that empowers through an ethos of co-production, personalisation and patient centered care. Establish networks and key stakeholder relationships throughout both the micro systems relating to existing pathways as well as the wider system across the West Yorkshire footprint and the wider region Bring together key stakeholders to influence and build a shared understanding of future models of care that promote patient centered, needs focused rehabilitation Present proposals that deliver efficiencies for the system as well as for the time of the patient Challenge health inequalities within existing pathways and ensure any proposal moves to address these Understand, support and ensure the representation of the patient voice throughout the project through co-creation Provide innovative clinical leadership and contribute to the strategic direction of the West Yorkshire Integrated Care Board in the management of patients living with Long Covid, ME/CSF and other Complex Conditions, utilising recent learning and research to propose innovative service development solutions.
